Hospital Discharge Support

It may be that there has been an accident or an unexpected discharge from hospital. If this happens, please contact Ringstead and see how we can help you.

If you or a loved one has been discharged from hospital but feel that help is still needed, then one of our friendly, helpful carers can make everything seem easier to cope with. Perhaps, the hospital will not allow you home unless there is help at home. If this is the case, then please give us a call.

A hospital stay can be a difficult time for both you and those close to you. No one likes to stay in hospital and always hopes the visit will be as short as possible. Ringstead can help by providing care at home when you are discharged.

The type of help needed when you are discharged will depend on why you were in hospital. Ringstead can help with the following but if you have other needs please ask:

  • Bathing
  • Dressing
  • Eating (are there diet restrictions, e.g., soft foods only? Certain foods not allowed?)
  • Personal hygiene
  • Grooming
  • Toileting
  • Transfer (moving from bed to chair)
  • Mobility (includes walking)
  • Using certain special equipment
  • Co-ordinating and overseeing medication
  • Household chores

As with all our services, A Person-Centred Active Support approach is used to make a specialised tailor-made care plan to fit in your preferences and would be reviewed frequently and therefore will be flexible, bespoke and will be fully discussed and agreed with you. The care plan will be regularly checked and adapted to your health progress at home.
